Permission Forms

A filled out & signed Troop Permission Form is required for every Troop activity. This acts as Parental Approval for the activity, a Medical Release in case of emergencies, it provides Parental & Backup Contact Information and a means to document any specific medication requirements or allergies/conditions and Parentally Acceptable OTC medications elections.

Best practice here is to fill out a copy of the form online, print it and save it to your hard drive for future reuse - just update the activity details! You will need to use Acrobat Reader 8 or later (available free from Adobe) to be able to save the content for reuse.
